This D2R Uber Tristram guide in short: the Pandemonium Event starts with three keys, Terror, Hate and Destruction, which open portals to mini-bosses that drop three organs. The organs open Uber Tristram, where you fight Uber Mephisto, Uber Diablo and Uber Baal. Max resistances, high life and strong damage are essential.
How does the Pandemonium Event work?
The Pandemonium Event is the hardest content in Diablo II: Resurrected, played in Hell difficulty. It has two stages. First you cube keys to open portals to three mini-uber bosses. Then you cube the organs they drop to open Uber Tristram, where three powerful versions of the Prime Evils wait. It is the same as of October 2026, in Ladder Season 15.
| Stage | You need | You get |
|---|---|---|
| Mini-ubers | Key of Terror, Key of Hate, Key of Destruction | A chance at the three organs |
| Uber Tristram | Mephisto's Brain, Diablo's Horn, Baal's Eye | The chance to beat the Ubers |
How do you collect keys and organs?
Keys drop from specific bosses in Hell difficulty, so farming them is a steady routine of short boss runs. With one of each key, you open the portals and fight the mini-ubers. Organs are not guaranteed from every kill, so expect several sets of keys to get all three organs. Keys and organs can also be traded between players on the same realm.
Plan your key runs as a short loop you can repeat in a few minutes, and keep spare sets in your stash so a failed attempt does not stop your evening. Hardcore players should treat every attempt with extra care, since a death there is permanent.
How should you prepare for Uber Tristram?
- Resistances: max fire, cold and lightning, and as much poison as you can.
- Life: high life, because the Ubers hit very hard.
- Damage: a build that can finish fights before your potions run out.
- Potions: a full belt and spare rejuvenation potions.
Many players do it with a group so one character can tank while others deal damage.
How do you beat the three Ubers?
Uber Tristram has Uber Mephisto, Uber Diablo and Uber Baal, plus their minions. Separating them is the standard approach: pull one at a time away from the others so you never face all three at once. Uber Mephisto is usually first, because his attacks are dangerous at range. Uber Diablo hits very hard, and Uber Baal summons clones.
- Clear the entrance. Make room to fight.
- Pull one Uber. Lead it away from the rest.
- Kill it cleanly. Use potions early, not late.
- Repeat. Take the others one by one.
What do you get for beating Uber Tristram?
Beating all three Ubers gives a chance at a unique charm that many endgame builds want. Drops are part of the game's normal rules, and nobody can promise a perfect roll. For the rest of the season's context, see our Ladder Season 15 guide.
Because every attempt costs a full set of organs, practising the fight with a stronger friend or an experienced group is worth it before you spend your own sets. Watching how they pull and separate the Ubers teaches more than any written guide.

D2R FAQ
What do you need for Uber Tristram in D2R?
You need three organs: Mephisto's Brain, Diablo's Horn and Baal's Eye. They drop from the mini-uber bosses you reach with the Keys of Terror, Hate and Destruction. Cube the three organs together in Act 5 of Hell difficulty to open Uber Tristram.
What resistances do I need for the Ubers?
Aim for max fire, cold and lightning resistance and as much poison resistance as you can get, plus high life. The Ubers hit very hard, so many players also use gear that reduces damage or steals life.
Can I do Uber Tristram solo?
Yes, with a strong build and good gear. Many players do it in a group so one character can hold the Ubers while others deal damage. Pulling and fighting one Uber at a time is the safest approach either way.
What drops from Uber Tristram?
Beating the Ubers gives a chance at a unique charm wanted by many endgame builds, along with other drops. Rolls and drops follow the game's normal rules, so no run guarantees a specific result.
Do you sell Hellfire Torches or organs?
No. We never sell torches, organs, keys, runes or items. For an Uber Tristram carry you bring your own keys or organs, and we help you win the fight in a game with you or on your account. Whatever drops stays with you.
